Please can you advise if ms sql server is included in windows 2012, my client would like to use us but he is running azure at a high cost. Or advise which windows 2012 version you offer on the top level 4 core windows kvm vps.

When you order KVM Windows server, you can choose to install Windows 2012 server OS, where you can configure it and add the necessary software yourself. Ms SQL Server is not included, unfortunately, but you can install it yourself. More information can be found in the official documentation here:
